Abstract:

Introduction: Hirschsprung disease (HD) is a congenital intestinal aganglionosis whose management is removing the affected colon, which pelvic dissection proximal to the female adnexal, a risk factor for infertility. Association with Mullerian malformations is infrequent in the literature, these represent 8% of the causes of infertility, and specifically, septate uterus (UT) is the third most common.

Case presentation: We present the case of a female patient with a history of Hirschsprung disease and secundary infertility due to a septate uterus. Exploratory laparoscopic sugery was performed, revealing multiple adhesions to the female adnexa. Therefore, resection of the vaginal, cervical and uterus septus, adhesiolysis, and bilateral tubal occlusion were performed due to the presence of bilateral hydrosalpinx. The patient had a favorable postoperative course and was discharged with the goal of undergoing in vitro fertilization (FIV).

Conclusion: This case highlights the importance of managing the couple by considering the factors that influence conception. In cases of severe adhesions and complications such as hydrosalpinx, bilateral tubal occlusion is a fundamental step in increasing the success rate of in vitro fertilization.
