Abstract
Introduction
The placenta accreta spectrum (PAS) corresponds to abnormal invasion of placenta into myometrial tissue. It includes placenta accreta, increta and percreta which will be differentiated according to the degree of penetration of placental tissue into the uterine wall. An incidence of 1 in 500 deliveries is described in developed countries, identifying several risk factors such as placenta previa, multiparity and previous cesarean sections, the latter being of great importance due to the increase in cesarean deliveries worldwide. The correct study and early identification of this entity is of special relevance for an adequate management of the possible complications it may have.
Case presentation
A 38-year-old female patient, with no morbid history, G5P4A1, 34+6 weeks pregnant, with obstetric history of four previous cesarean sections and a missed abortion requiring manual vacuum aspiration (MVA), attended the emergency department for scheduled surgery due to a history of placental accretism.
A history of transvaginal ultrasound showed total occlusive placenta previa, intraplacental lacunae, loss of myometrial continuity, increased placental interface vascularization and utero-vesical interface discontinuity.
The surgical approach for 35 weeks was planned together with the patient, explaining the possible risks and complications. A protocol was drawn up, which included the participation of gynecology, urology and anesthesia, with the availability of blood products and space in the intensive care unit.
In the ward, placental percretism was confirmed and a cesarean section, obstetric hysterectomy and cystorrhaphy with left ureteroplasty were performed secondary to placental tissue invasion; the patient presented a total bleeding of approximately 6500cc, requiring transfusion of blood products.
Discussion:
PAS represents an obstetric challenge, having a wide range of presentations and complications such as massive postpartum hemorrhage, requiring high index of suspicion, timely diagnosis and a multidisciplinary confrontation before eventual complications.
Key words: Placenta Accreta, Postpartum Hemorrhage, Hysterectomy, Pregnancy, Uterus.
